New Patients

We'd like to welcome you to our office. We are pleased that you have chosen us to provide you and your family's eye care needs. Our office is conveniently located in downtown Stamford. A detailed map is available on the home page if directions are needed. Free parking is available in our back lot or on the street in front of the building.

To make your visit with us go smoothly, please bring the following with you:

1. Medical Insurance Card
2. Vision Care Plan Card
3. Patient Registration form filled out and signed HIPPA acknowledgement form, both of which can be downloaded on the Forms tab on the homepage.
4. A copy of your most recent eyeglass or contact lens Rx.
5. Any medical info that will be pertinent to your eye exam.

Please try to arrive 5-10 minutes early and be aware that eye drops may be applied to dilate your pupils which may blur your vision for reading for 2-3 hours. You will also be light sensitive so bring sunglasses or we can provide a disposable pair.
