You have the option to complete the new patient process online. This option lets you enter your patient and medical information online.

Option 2: Printable Form

If you have a printer, you can print out our new patient form and our medical history form. After printed, bring the completed forms to our office on your initial visit.

Click on the links below to access Patient Forms. You can print or save them to your computer. New patients please print, fill out and bring with you the Patient Registration, read thoroughly the Notice of Privacy Practices and print, fill out and bring with you the HIPPA Acknowledgement. We also request you bring your sunglasses, boxes or sleeves of current contact lenses (if you have these) and a list of medications you currently take.
